Before you schedule an appointment with a nose specialist, the first question on your mind is usually: “Will my health insurance cover my surgery?” Navigating the endless details contained within the pages of your health insurance plan can be overwhelming and frustrating. Understanding even the most basic concepts of an insurance policy can be clouded by fine print and exceptions. Dr. Bennett has created this online guide to help current and potential patients learn the ins and outs of your health insurance policy, to ensure that you get the best care for the lowest out-of-pocket expense. The first question you must answer when evaluating your health insurance plan is figuring out what type of health insurance you have. There are two major types: an HMO plan and a PPO plan. There are many hybrid versions of these including EPO, POS and OA+. Health Maintenance Organization (HMO) Plans HMO stands for health maintenance organization. Here, a monthly payment allows you to see any doctor within the HMO group. However, seeing a doctor is not as seamless as making an appointment. Before your HMO can cover your consultation, you must first meet with your PCP, or primary care provider. Your primary care physician is your normal doctor that you see when you need a check-up, or when you need advice or treatment concerning general medical issues. You will speak with your PCP about the type of HMO specialist you wish to see, and your PCP will decide whether you in fact qualify for the appointment. For example, if you suffer from sinusitis, before you can see a sinus surgery specialist you must first meet with your regular doctor, who may approve you for the consultation with the [...]

The Guardian Life Insurance Company of America (GLICOA) was founded in 1860 in New York City. One of the largest mutual life insurance companies in the U.S., it is ranked as a Fortune 300 company. The company boasts over 5,000 employees, 3,000 representatives and 80 branches across the country. Policyholders own the company, which has a reputation of keeping a close eye on its services and investments. Guardian is a versatile insurance company. Employers can tailor benefits packages to their employees’ needs, selecting from a HMO, POS or PPO plan, with the optional inclusion of indemnity coverage. GLICOA is most commonly associated with dental insurance, however the company also offers an assortment of other insurance and annuities packages. Guardian subscribers have access to life insurance, disability, group and individual medical and vision packages, 401(k) plans, income insurance and retirement programs. The company is split into two separate smaller entities: Guardian Healthcare, which oversees the management of medical insurance and similar services; and Guardian Investor Services, that controls securities and annuities. Guardian recently took efforts to streamline their operations and assist customers in new and important ways. Many policies now include erratic and unforeseeable events that were once a threat to coverage – such as hospitalizations and pre-existing/chronic conditions. A merger with Destiny Health Plan allows Guardian to help customers reduce costs and get the most out of their coverage via a “consumer-controlled saving account.” Over 120,000 American employers chose Guardian to provide benefits to nearly six million employees and their families. Managing your own care with any insurance company can be daunting. We have found that Guardian Health Insurance has better customer service than many others, and will strive to take excellent care of their [...]

Oxford was formed in 1984 and merged in 2004 into United, the largest healthcare corporation in the United States. Oxford Health Plans serve the New York, New Jersey and Connecticut areas. Available plans provide traditional health care benefits to employers and individuals. Oxford is a for-profit company. Oxford frequently changes its 11 page rhinoplasty, septoplasty and vestibular stenosis clinical guidelines. Where insurance is renewed annually, it is not uncommon for Oxford to revise its policies twice or even three times per year. The insurance also has a disclaimer that: “Oxford reserves the right, in its sole discretion, to modify policies as necessary without prior written notice”. The last nose clinical update as of this article was in April of 2013. Dr. Bennett’s office is experienced with Oxford Health Plans, and can familiarize you with facts about your eligibility and scope of benefits. As an out-of-network provider, we are able to spend more time with you to explain your care so you can feel more confident in your treatment. We will also give you detailed information about the claims referral process and precertification status. We are committed to simplifying your claim and paperwork requirements. Let us do the work, so you can focus on getting healthier and improving your overall quality of life.

UnitedHealth Group Incorporated is the parent of United Healthcare, the biggest health insurance company in the United States. Fortune magazine ranks UnitedHealth number 22 on its Top 500 American Companies list. Its organizational structure includes numerous subsidiaries and divisions that in total serve more than 70 million people nationwide. A large membership group partially accounts for the company’s average yearly earnings of over $5 billion. United traces its roots back to 1974. The company developed swiftly after several acquisitions, and was the first company to create network health plans for seniors. In 1984, United changed its enterprise status to a publicly traded, for-profit company, listed on the New York Stock Exchange. If you are a United policy holder, you may be curious about the details of your plan. Will it cover the type of nose surgery that I need? Will I need to pay any out-of-pocket expenses? Do I have a deductible? Health insurance consumers are accustomed to changes in premiums and other rates. However, it can become especially confusing if this information is not communicated in an easy-to-understand way. United Healthcare has had 8 changes alone in its 11 page nasal surgery policy since it was rewritten in December of 2010 with corrections in May of 2013 replacing the revision from on April of 2013. Your insurance policy is only renewed once per year. We make a point to stay updated on each revision to make sure you know what to expect. If you suspect you may need nose surgery, and are a United member, schedule an appointment at our upper east-side practice. As an out-of-network office we can spend more time and attention with your care than in-network practices that may be required [...]

GHI (Group Health Incorporated) was established in 1937 as a non-profit health cooperative. The insurance provides health care to those Americans who teach our children, protect our country, and provide services to our community. Currently, GHI is the biggest non-profit insurance company in the state of New York, with over 2 million members. GHI is run by EmblemHealth and has submitted a proposal to become a for-profit publicly traded corporation. GHI continues to have relatively low co-payments for medication and office visits. However, it seems that over time, benefits continue to decrease and patients find it difficult to find a qualified physician who not only takes their insurance, but also connects with them. Finding a surgical specialist may prove to be even more difficult. Our office will work with your insurance to help you get the most from your benefits. As an out-of-network provider, we realize that patients have financial hardships and will do our best to make your procedure affordable. We don’t believe that those who serve our community and teach our children should be forced to go into debt in order to receive the care they deserve. Navigating the maze of health-care is extremely challenging and dealing with the complexities of insurance paperwork can seem like an impossible web to untangle. Our highly skilled insurance staff will check your benefits for you and describe your benefits to you in detail. We can’t work with HMOs but have had great patient experiences with the other GHI insurances.

The Empire Plan and the New York State Health Insurance Program (NYSHIP) have been providing state and local government employees with wide-ranging health insurance for decades. It is the largest public employer health insurance program in the United States, with 1.2 million insured. Enrollees that receive benefits include: active employees, retirees and their dependents and dependent survivors. NYSHIP was first founded in 1957 and was available only to employees of the state. The next year, it introduced policies available to local government employees and local school districts. Three decades later, in 1986, NYSHIP created The Empire Plan, making insurance available to an even wider group of public employees. This growth allowed The Empire Plan to become the reliable and unique insurance company that New Yorkers know today. In 2013, there are currently 800 public employers that use the Empire Plan. Unlike most of NY state for-profit insurances, the rates for The Empire Plan have not seen large changes. The annual net premium increase over the last five years was only 4%, on average. Patients also have access to a network of over 190,000 providing medical doctors, surgeons and specialists from which to choose. The NYSHIP Empire Plan supports New York public employers beyond just local government agencies. The plan covers teachers in public school districts, librarians, staff at public libraries, fireman, public park workers and those employed by utility companies. Dr. Bennett and his staff view these workers as an invaluable part of our local community and of the state of New York as a whole. They are deserving of the best possible experience from the initial consultation to the recovery period. The Empire Plan is known for its reputation as an all-inclusive and affordable [...]

Aetna Incorporated is one of the largest health care companies in the United States. Originally known as Aetna (Fire) Insurance Company, of Hartford, Connecticut, the corporation provides a wide variety of health insurance products. Aetna currently offers its subscribers several different types of insurance, including medical, dental, long-term care and disability, and behavioral health plans. It is one of the only insurance companies with membership in the Fortune 100. More than 37 million Americans have medical coverage with Aetna. The company serves a wide variety of customers, including part-time workers, employer groups, individuals and families, college students, governmental workers and labor unions. Over 1 million health-care professionals are members of Aetna with nearly 500,000 primary-care doctors also having membership with the company. Aetna is a for-profit insurance company. If you subscribe to a large insurance carrier like Aetna, obtaining accurate information and resources about your policy can often times be a challenging and frustrating process. The Aetna “Septoplasty and Rhinoplasty Clinical Policy Bulletin” is lengthy and filled with medical terminology. Your insurance is issued every year but this policy has been revised/updated 6 times in the past 3 years alone. In addition, there are brochures outlining policy “limitations and exclusions” to sinus surgery, septoplasty and other nasal surgery that may not be obvious until a claim is denied. Dr. Bennett and his team strive to ease patient’s minds by providing you with a personalized touch. As an out-of-network office, Dr. Bennett does not overbook patients and is able to spend as much time with you as needed.
Steam Vaporizers & Your Sinuses A steam vaporizer is type of humidifier that converts water into a warm, moisturizing mist to combat the harmful effects of dry air on the sinuses. Sinus infections, whether acute or chronic are associated with a number of painful symptoms, including nasal congestion and inflammation, sore throat, fever and sinus and facial tenderness. Your daily routine and social life can be rudely interrupted when you are suffering from a sinus infection. Managing the delicate tissues in your nasal membrane to prevent bacterial or viral infections is key to preventing sinus infection symptoms. There are many methods of treatment for improving sinus health. Vaporizers are special in that they are a safe, natural and ongoing remedy. Vaporizers also allow you to add certain beneficial oils that strength your immune system and offer you soothing relief. Simply add a few drops of extract into the water basin and turn on your vaporize. Within an instant, the air will begin to fill with a refreshing air that you can inhale deeply, causing your dry and blocked sinuses to transform into a well-moisturized, clear airway. You can find eucalyptus or lavender oil in your local drugstore. Dr. Bennett’s Helpful Steam Vaporizer Tips Because warm-mist vaporizers use steam to create moisture in the air, be sure to keep the machine out of the reach of children, as it could cause burns when handled by small, delicate hands. Sleeping in an elevated position will decrease nasal congestion. Place a few extra comfy pillows behind your head and allow any blocked or infected mucus in your sinuses to drain with ease. You can use tap water [...]

Introduction During the cold, dry, winter months, the lack of moisture in the air can make you more prone to sinus infections. Dry air can bring irritating and uncomfortable symptoms that can disturb your home and work life. Prevention is an important part of maintaining healthy sinuses. The installation of a humidifier in your home or work space is one of the best preventative tips to follow. A humidifier adds moisture to the air. A steam vaporizer is a type of humidifier that adds warm moisture to the air. Medical experts are divided on which type of humidifier is the healthiest. The terms humidifier and vaporizer are generally used interchangeably as devices that put moisture into the air. Dr. Bennett recommends using a humidifier to moisturize your sinuses and breathe easier. Dr. Bennett’s Guide to a “Sinus Friendly” Home series gives you essential need-to-know facts about these machines. Regardless of which type of humidifier you use, remember that cleaning your device is extremely important, as it will not only allow the machine to run more effectively, but will also prevent contamination. Also remember that while the most important time to run your humidifier is in the winter or in the dry early months of spring and late months of fall. How does a humidifier prevent sinusitis? During certain times of the year, particularly the winter months, the air is dry both indoors and outdoors. Inhaling this dry air may have adverse effects on your sinuses, including dry, cracked nostrils, thicker mucus, blocked sinus passages, and general irritation.
